    Nexus Glen Lake, a small intensive residential treatment program located in Minnetonka, is looking for a dedicated Quality Improvement Coordinator to support and collaborate with the various disciplines and functions of Nexus Glen Lake and Nexus through guiding the collection and analysis of quality data to inform decision-making at the client, program, and systems levels.  This role will also ensure all licensing and regulatory agency requirements are satisfied and that Nexus standards for treatment are being met.

    Responsibilities

    Responsibilities include:

    • Develop and oversee all quality improvement (QI) functions and activities for the residential facility.
    • Review all reports of crisis interventions, incidents, client complaints and grievances, allegations of maltreatment, and other unusual incidents; investigate, and partner with departments to recommend solutions.
    • Ensure agency compliance with all Medicaid, Council on Accreditation (COA), PREA regulations, and other standards, policies and licensing requirements.
    • Facilitate incident investigations and analyses and assist with projects associated with QI goals and strategies.
    • Determine areas of education and training needed by the site staff surrounding the maintenance of standards of quality documentation and processes; assist in the development of training programs, as well as provide training, as requested.
    • Lead Nexus Glen Lake’s Risk Management and CQI Committees and participate in the Nexus Quality Improvement Committee and other committees as assigned.

    Qualifications

    QUALIFICATIONS (required unless otherwise noted)

    • Bachelor’s degree in Human Services or related field required; MA degree in Human Services (or related) preferred.
    • Combined minimum of 3 years’ residential and/or QI related experience required.
    • Minimum of 1-2 years of experience in QI/QA, outcomes management, program evaluation, or residential treatment preferred.
    • Solid understanding licensing rules/regulations, regulatory, accreditation and related government agencies and in quality improvement terminology and practice.
    • Demonstrated competency in quality improvement processes and helping stakeholders access and use information meaningfully through thoughtful and cohesive data analysis and visualization.
    • Proficiency with MS software (including Excel), along with intermediate to advanced skills for creation of spreadsheet and data collection systems.
    • Ability to sit for extended periods of time and utilize computer terminal, keyboard, and assorted office equipment and to twist, bend and stretch in a manner conducive to performing daily activities in a residential treatment facility.  Also requires visual acuity conducive to extensive computer monitor work.
