SOCIAL WORKER - full-time Dickinson PATH office.
The social worker recruits, orients and supervises the work of PATH resource families while
providing intake services and comprehensive case management for children with special needs
DUTIES AND RESPONSIBILITIES
(The following list is not comprehensive but highlights the major areas of responsibility for the position.)
1.Recruit, license, train, supervise, and provide ongoing assessment to resource families to ensure quality care consistent with regulatory agencies (i.e. DHS, COA, FFTA).
a. Participate in the development and implementation of local, regional and state plans for recruiting and retaining resource families.
b.Follow state and agency guidelines for conducting family/homestudy, physical environment inspection and criminal background checks and provide required documentation for initial and ongoing licensing.
c.Coordinate required orientation and ongoing education and skill development for resource families.
d.Supervise resource families’ interaction with foster child, child’s family, treatment team members and collateral contacts.
e.Monitor and evaluate resource families’ activities to ensure compliance with agency and state standards. Submit critical incident reports and develop and implement corrective action plans as required.
f.Facilitate required team/unit meetings and participate in other resource families related activities.
g.Meet requirements for foster home visitation.
2.Provide comprehensive coordination of all tasks required to meet the treatment needs and best interests of service recipients consistent with regulatory agencies (i.e. DHS, COA, FFTA).
a.Comply with local office procedure for referral and service delivery.
b.Complete a systemic assessment of the needs of the child and family.
c.Establish service parameters with legal custodian as child’s needs dictate.
d.Provide complete and required documentation, within established time lines relating to case management and treatment planning.
e.Implement treatment services to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family.
f.Provide direction to other team members when applicable and make recommendations for discharge, finalization and permanency.
g.Promote the needs of youth and families to ensure successful outcomes.
h.Meet requirements for youth/family contacts.
i.Advocate for the development and maintenance of funding and resources to meet youth and family needs at local, state and national levels.
j.Develop and maintain community contacts to promote PATH and collaborative efforts.
k.Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and foster family.
3.Conduct job duties from a framework that acknowledges the impact of childhood trauma on children, caregivers, and service providers. Integrates trauma informed care principles into practice by sharing knowledge and understanding of trauma with the child, foster family and relevant team members. Additionally, utilizes and advocates for the use of trauma-sensitive strategies to support the youth and their family.
a.Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision.
b.Routinely makes resources available and shares knowledge with foster families and providers on trauma exposure, its impact, and trauma-focused treatment.
c.Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma.
d.Emphasizes continuity of care and collaboration across child-service systems.
e.Understand the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects.
4.Meet standards for productivity and caseload size while maintaining appropriate documentation and files to support the services that are provided to families.
a.Complete paperwork and compile documentation required by PATH and governing/accrediting agencies; maintain current files on all youth in placement.
b.Produce informal and formal reports as requested by parents, referral agencies, courts, etc.
5.Participate in staff and supervision meetings, and required PATH in-service meetings. Understands and supports the PATH and Nexus philosophy and mission.
a.Participate in individual and group supervision including clinical consultation to review cases and ensure best practices are provided to all children and families.
b.Provide timely and thorough information to supervisor regarding case incidence, service delivery struggles and difficult team processes.
c.Utilize supervision options and directives in service delivery.
6.Ensure confidentiality of all treatment and services information pertaining to clients and their foster, adoptive and biological families.
7.Perform other job related responsibilities, tasks and projects as needed.

Education, Credentials and Experience
Social Worker I:
o Bachelor’s degree in Human Service related field required.
o Current social work licensure.
o Experience in foster care, adoptions and/or family-based service preferred.
Social Worker II:
o Bachelor’s degree in Social Work required.
o Current social work licensure required.
o A minimum of two years’ experience in foster care, adoptions and/or family-based service required. In lieu of experience, a Masters in Social Work will be accepted.
Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards.
