THIS POSITION FULL TIME CHAFFEE INDEPENDENT LIVING COORDINATOR
Location: Serving Devils Lake and Turtle Mt, the successful candidate may be housed at the Devils Lake or Turtle Mt PATH office.
Through the Chaffee Independent Living Program the Coordinator assesses, develops and conducts skills training for eligible individuals, works with community partners to provide services and opportunities, and advocates for resources to assist foster youth ages 16 and older in successfully transitioning youth to adulthood.
Coordinate Assessments and Services:
Provide comprehensive coordination of all tasks required to meet the needs and best interest of service recipients consistent with DHS, COA, PATH mission, values, policies, ethics and practice.
1. Identify youth who are age 16 and older, and who are likely to remain in care until the age of 18.
2. Administer assessments and develop plans.
3. Develop and conduct a relevant and effective program;
4. Verify eligibility for Chafee Funds;
5. Assure that Chafee expenditures are allowable;
6. Assure that supplantation of funds does not occur;
7. Identify and document available match dollars;
8. Assist eligible young adults with the ETV process;
9. Prepare an annual plan and budget for your Region’s Chafee Program;
10. Utilize the wraparound process in developing, implementing and monitoring the Independent Living plan for the service recipient.
11. Facilitate montly youth groups on Independent Living.
Data management / Reporting
Ensure that documentation is thorough and timely as required by standards.
1. Provide documentation of flex fund expenditures on a monthly basis.
2. Conduct and document diligent efforts to obtain follow-up data on youth who have exited the program;
3. Respond to requests for data from the state coordinator;
4. Provide quarterly and year end reports to state coordinator;
Promote the needs of service recipients to ensure successful outcomes.
1. Advocate for foster/former foster youth; encourage youth to be self-advocates and leaders.
2. Advocate legal rights for service recipients.
3. Advocate for resources to assist service recipients to successfully achieve Independent Living goals.
4. Educate the community about the needs of service recipients.
5. Advocate for the development, maintenance of funding or services to meet youth and family needs at local, state and national levels.
6. Develop and distribute written materials to eligible participants.
Represent PATH’s mission and values with integrity in all interactions.
1. Develop a good working relationship with eligible youth and young adults, their caregivers, custodians, informal supports and social workers, using a positive youth development and wraparound approach;
2. Include input from foster/former foster youth in program design and implementation.
3. Partner with the broader community to provide a supportive learning environment for foster/former foster youth, which may include engaging community partners in mentoring, job placement, providing tangible supports to the Chafee Program, sponsoring youth, educating youth about subjects such as banking, credit, car purchases, comparison shopping, and other life skill areas;
4. Utilize feedback to improve PATH practices and services.
Supervision and Practice
1. Participate in individual and group supervision to review cases and ensure best practices are provided.
2. Utilize supervision options and directives in service delivery.
3. Provide timely and thorough information to supervisor regarding case incidents, service delivery struggles and difficult team processes.
4. Consult with the state coordinator as needed;
5. Participate in required training and meetings;
Participate in PATH meetings, training, quality improvement, special projects, teams and duties as assigned and demonstrate positive spirit and teamwork in the workplace.
Education, Credentials and Experience
•Masters or BA degree in social work, psychology, vocational rehabilitation, education, child development, or other behavioral field.
•Two or more years of experience working with youth in foster care and/or behaviorally challenged youth, and/or emotionally disturbed youth, and/or youth involved in the juvenile justice system, and/or youth within the educational setting.
•Proof of valid driver’s license, auto insurance and access to vehicle capable of safely transporting clients.
•Successful completion of background checks and all applicable health requirements.
•Flexibility to work varied hours, as needed to meet the needs of those served
