Support and reinforce the therapeutic treatment plan of clients by providing guidance and direction to residents in their daily living activities, and by coordinating internal and external resources and activities, along with maintaining records for an assigned case load of residents.


1. Manage and coordinate resources and activities for each assigned client to ensure that the unique treatment needs and requirements of each individual are met.

a. Coordinate, provide transportation and facilitate/co-facilitate case conferences and client-oriented team meetings with both internal and external resources including county or state referral sources, court appearances, scheduled family visits, etc.

b. Active participation in the development and implementation of individual treatment plans, compile resident assessment materials, maintain required clinical documentation and report within provided deadlines.

c. Collaborate with and maintain regular contact with family members, licensing, parole officers, the court system and other organizations to ensure ongoing consensus on therapeutic treatment direction, behavior expectations, progress and outcomes.

d. Accurately maintain individual clothing accounts and other residential records as required and ensure basic needs of clients are met (i.e. adequate clothing).


2. Provide mentorship and supervision of resident daily activities and behavior according to established guidelines and procedures.

a. Ensure that residents adhere to daily schedules and activities established for the program, including school, therapy sessions, external appointments, etc.

b. Ensure that residents comply with required activities surrounding personal hygiene and grooming, upkeep of living areas, laundry, etc.

c. Serve as a mentor and role model by demonstrating compassion and understanding for feelings, opinions and experiences of others, and communicating and interacting in a manner that is honest, positive, respectful and professional.

d. Demonstrate an acceptance and respectfulness toward diversity concerning commonalities and differences such as gender, race, age, culture, ethnicity, class, religion, disability and gender identity.

e. Administer medications per established protocols and generate related recording requirements accurately.


3. Participate with coworkers as a team on implementation of treatment plans and therapeutic services.

a. Participate in all required meetings and conferences relative to assigned clients.

b. Remain cognizant of individual resident levels/phases, privileges and restrictions and adapt interactions accordingly.

c. Engage in therapeutic interventions with residents within treatment parameters, while maintaining appropriate boundaries.

d. Report to supervisor signs and symptoms potentially requiring intervention or referral (suicidal or violent tendencies, self mutilation, eating disorders, etc.).

e. Attend staff meetings regularly and actively participate.


4. Ensure completion of all required documentation standards including, but not limited to, critical incident reporting.


5. Ensure resident safety and collaborate with coworkers to maintain an environment that is positive, respectful, clean and therapeutic.

a. Ensure compliance with all safety policies and procedures involving living areas, buildings, campus grounds and while attending off-campus activities.

b. Perform First Aid/CPR procedures and respond according to established protocols in the event of an emergency situation.

c. Maintain appropriate boundaries in all interactions and contact with residents.

d. Comply with Nexus policies involving privacy and confidentiality for residents and their families.


6. Assume Staff-in-Charge role when appointed.


7. Perform other responsibilities, tasks and projects as needed.


Knowledge, Skills and Personal Attributes

  • Remain current on all required training and certifications.
  • Demonstrate sufficient written and verbal communication skills to generate required documentations and reporting that is legible and articulate, and to interface professionally and effectively with multiple external contacts, families and staff.
  • Demonstrate ability to utilize organizational skills and tools to proactively manage multiple client treatment needs, appointment schedules, case meetings, court visits, etc.
  • Exhibit ability to lead and provide work directions by example and through mentoring in a manner that is patient, positive, respectful, non-threatening and support of Nexus methods, culture and values.
  • Exhibit a spirit of cooperation among coworkers and comply with corporate and school policies and procedures.
  • Display ethical conduct and professionalism at all times, with particular emphasis on maintaining a healthy relationship and appropriate boundaries with students, and protecting student confidentiality.
  • Adhere to department procedures regarding punctuality and attendance and exhibit flexibility and cooperation with shift scheduling and rotation needs.
  • Strive to gain an understanding of basic child and adolescent development principles, in an effort to model and reinforce fundamentals of positive youth development.

Education, Credentials and Experience

  • Bachelor’s degree required, 1 year of related experience strongly preferred.
  • Minimum 21 years of age for all clinical program serving clients older than 12 years of age.
  • Illinois: Must meet Medicaid requirements for a Mental Health Professional (MHP).
  • Valid driver’s license required. Must meet state driving criteria and pass corporate eligibility standards.


Physical Demands

  • Must be able to perform physical interventions and restraints of residents.
  • Must be able to lift up to 50 pounds unassisted and up to 100 pounds with assistance.
  • Must be able to stand and walk for extended periods of time (2-4 hours).
  • Must be able to twist, bend and stretch in a manner conducive to the execution of daily activities within a residential treatment facility.
